It is important to understand, why stress is on the human breast milk. Nature has made provision that every mother’s care is just right for her baby.

Thus, cows’ milk is just right for the calf, goat’s milk for baby goat, lioness milk for the Lion cub, cats milk for a kitten and the list goes on, ending with human milk is for the human baby.

In fact, the success and Cost of the IVF cycle depends upon this.

Congratulations are due as you have crossed the 1st milestone – the fertilization or fusion of healthy eggs harvested from the woman with sperm. The sperm may be from her husband or from a source unknown to the woman.

In the course of the natural fertilization process, when the embryo is formed within the body, changes occur inside the uterus as it prepares itself to accept the embryo and subsequently offer it nutrition. This seemingly simple process is actually a very complex one.

During the IVF cycle, this complex process is carried out by the embryologist in the IVF laboratory.  This entire process needs to be monitored while creating conditions as close as possible to those in the woman’s body.

Between 24 and 30 hours after fertilization (Day 1), the embryo gets divided into 2 cells. By Day 2 the embryo is 4 cells and by the third day, the embryo is a 6-8 celled structure.

On the 4th day, the embryo cells are now fused, within 16-32 cell structures. This structure is now called a morula, which is a fast developing stage.

On day 5 of the embryo, the cells are specialized and carry out necessary functioning. The outer surface of the morula is the placenta in making & fluid secretions turning into the amniotic fluid.

Now, the real challenge is surviving and growing during the next 48 hrs. (day 3 to 5). The not-so-competent embryos will not survive this stage & leave only the best to be selected for transfer. In the womb, implantation will occur only if the embryo reaches the day 5 stage.

The IVF center has the option of transferring on Day 3, hoping that the natural environment within the womb will take it to day 5 and enable implantation to occur.

However, this is taking a risk at the patient’s expense. If, after a day 3 transfer an innately deficient embryo does not survive to grow to day 5, the transfer is bound to fail.

At Pearl Women’s Hospital, we do only day 5 transfers. In short, we transfer only when we are confident that the embryo is good enough to have grown up to day 5, increasing the chances of successful implantation.

In many institutes that offer low-priced cycles, the compulsion to reduce cycle rates means the adoption of cost-saving measures like bulk transfers. Also, many simply do not have an in-house embryologist. Thus, transfers take place only when the embryologist is available, which may be day 3 or day 5.

We are a professional institute with an in-house embryologist who is an M.Sc in Embryology from UK, a very rare commodity. No wonder then that our success rate is higher (71%) and therefore the cost to pregnancy is lower since we require less number of cycles for ensuring conception and live birth.

There are a few instances, where some women will not have any embryo left for transfer despite appearing good on day 3. It is very easy to say, such embryos which did not survive, eventually could not achieve pregnancy, even though transferred on day 3. Such embryos are chromosomally not competent.

Safe to say that the more advanced the growth of the embryo, the better the chances of implantation in the uterine lining.

To summarize – We transfer the embryo on day 5 only after its survival and quality have been confirmed. This increases the success rate of the cycle. This means the cost to pregnancy gets reduced since a lesser number of cycles may be required for pregnancy.

Fresh or frozen embryo transfer: which is better in the true sense?

The success rate is found to be higher with frozen embryos as compared to fresh embryos. Frozen cycles tend to have a higher success rate as the embryos that survive the thawing process are most adaptable, resulting in higher chances of achieving a pregnancy.

 

Fresh embryo transfer happens 5 days after egg retrieval, there is a shorter span, which is of 5 days for conception. Doctors prefer to look into the progesterone level at this stage. An elevated progesterone level will not allow embryo implantation.

 

Frozen embryo transfer typically happens 6-8 weeks after the embryo is frozen. This frozen embryo can have better chances to develop in a baby due to the favorable hormonal condition. Newer techniques in the preservation of embryos have an added advantage.

 

Likewise, Donor eggs if, obtained from, a younger donor who is medically fit, increases the chances of success rates. Also, better sperm quality always ensures a higher chance of conception.

The ageing of the father also has an effect on fertility because it affects the quality of the sperm, by causing damage to the Sperm DNA, thereby reducing the capacity to fertilize as well as the quality of embryos. Is IVF an established technology?
The year 1978 heralded a break through in infertility management when assisted reproduction technology (ART) brought into the world the first IVF (in vitro fertilized) baby, or Test tube baby as the process was then known. 25th July 1978 was the day on which, Doctors Patricks Steptoe and sir Robert Edward had the honour of bringing into the world Louis Brown, the world’s first IVF baby.

Simply put, IVF  refers to a baby made outside the body and then put into the uterus to grow for 9 months. However 44 years after the first IVF baby was born, the process of IVF remains an enigma, many families still believing that IVF babies may not be normal, may not be genetically theirs, and may stand apart from the rest of the babies.

It is important to understand that IVF or as it is commonly called even today,Test Tube baby,  is nothing but Assisted Reproduction Technology (ART).
The male and the female partners both contribute towards the making of the little new wonder in their unique way. The male partner contributes only the sperm & the sperm is the motile component of the whole procedure. Therefore when trying to conceive the male partner is asked to do a Semen analysis, which gives an idea of the quantity and quality of the sperms.

The quality of the sperm is determined by it’s motility, that is it’s ability to move ahead speedily, and it’s morphology – the appearance of the sperm. There are unfortunate cases where the semen has no sperm in it (Azoospermia) or the quantity may not be sufficient enough to achieve conception (Oligospermia). This can be due to  either no or low production or no or poor supply. This however needs to be investigated to find the correct reason.

Every male, however, needs to understand one very important point. That the ability to perform sex and the ability to impregnate are different aspects.  Meaning that even if the male is unable to perform sex due to impotency (erection failure), he may still be able to cause conception by resorting to IVF technology.

The female partners play a major role. This is because along with her gametes she also provides the lodging boarding facility to the growing baby for 9 months.  Like male counterparts, both the quantity and quality of the ovum or eggs matter. Since the ovaries are hidden inside the body, indirect means like sonography and blood tests need to be resorted to, to get an idea of the situation.

There are three categories of eggs.
The normal range ovaries (A), over-enthusiastic ovaries (B) & tired ovaries. (C).
The B and C categories are the ones which need more care & management than type A. The quality of eggs cannot be commented upon till visualized. However, we can safely say that the quantity and quality islikely to be good in A and may be compromised in B and C.

When two factors are responsible for a result, they must come together.
The sperm needs to reach the egg, get fertilized and then the resultant embryo needs to get back to the uterus, be nurtured and nourished, enabling it to grow well, eventually resulting in a mature baby at the end of the gestation period.
The path taken by the sperm and subsequently by the embryo after fertilization, is through the fallopian tubes. These are tunnels originating from the uterus ending in a hood over the ovary. They possess minute hair-like structures called Fimbriae, to push the embryo towards the uterus.

There are times when everything seems so right but yet nothing works right.
Therefore, if there is an absence of pregnancy even after a year of persistent and meticulous attempts then it is advisable that the couple seeks the help of assissted reproduction technology.

It rather means that help is available and it should be availed, just as it is for any other medical problem with human shortcomings.

Difficult to believe, isn’t it, that male infertility is almost as frequent an occurrence as female infertility? But true nevertheless. It means that responsibility for becoming parents lies almost equally with both partners and they have to work equally effectively to achieve their goal of starting a family.

 

It helps if both lead a lifestyle that is healthy and full of activities that will negate the stress of personal and professional commitments. Healthy eating, remaining physically fit, especially avoiding putting on unnecessary weight, and avoiding drinks & tobacco in all forms is a prerequisites when you are looking at commencing a family.

Then, the team performed a DNA analysis of the samples for risk of cancer. The researchers found a detrimental change in the DNA of babies whose fathers smoked.”

What it simply means, is that smoking has a powerfully negative effect on a baby’s health & more risk children carry for such diseases.

The smoke from cigars has the potential to the health of the sperm. Cigar smoke can alter the sperm structure, and motility and make them less competent. These epigenetic changes cause serious concerns like leukaemia and heart problems for children born to fathers who smoke.

There is no safe limit for smoking & every puff has the potential to damage the health of self, others with passive smoking & the unborn child, and lifetime damage to vital organs.

Quitting is not merely a decision, it is a lifetime gift of health.

3. Obesity:

It is a well-researched   & known fact that obesity increases the load on all organs equally & sperms structure. The research found that overweight men were 11 % more likely to have a low sperm count and 39 % more likely to have no sperm in their ejaculate. Obese men were 42 % more likely to have a low sperm count than their normal-weight peers and 81 % more likely to produce no sperm.” March 2012 Archives of Internal Medicine.

 

Obesity can increase the risk of infertility. Male obesity impacts negatively the reproductive potential by not only decreasing sperm quality but also altering the physical and molecular structure of germ cells in the testes. It simply means the effect of obesity is beyond maturation. Such sperms can create many abnormalities in the progeny.

 

Obesity by and large is an imbalance of energy consumption & energy taken by the body. Dietary modification & exercise helps in this regard. The guidance of a nutritionist is a prime requirement as losing fat without any fatigue is more important.

4. Environmental toxins: avoid them when you can’t control them.

Environmental exposures play an important role in disease orientation & development. In the case of pregnancies, the developing fetus is susceptible to environmental pollutants.

Such factors are, air pollution, tobacco smoke, pesticides, solvents, metals, radiation, water contaminants (disinfection by-products, arsenic, and nitrates) and chemicals (persistent organic pollutants (POPs) greatly impact pregnancy outcomes such as pregnancy loss, stillbirth, fetal growth, preterm birth and congenital anomalies.

Does every patient require IVF?

So, Let’s dive straight to the answer, absolutely no, Not every patient needs In Vitro Fertilization (IVF).

While elaborating on the topic, Does every patient require IVF, infertility concerns, age, medical records or past history have its importance.

Only infertility concern is the most evident & all are eager to find out the success attached with it..  Every case of infertility is unique. In simple terms, Infertility is the difficulty in conceiving a pregnancy after trying unsuccessfully for a year or so without contraceptives. This does not address the underlying cause or nature of infertility, as infertility could be temporary or permanent in nature.

Infertility is not an issue, it has many medical conditions as the underlying causes & is treatable if diagnosed well in time. Infertility, like many other medical diagnoses, can be of varying severity, like mild, moderate & severe ones.

Women who are struggling with pregnancy, face a wide range of emotional outbreaks & this stress is heartbreaking and impacts every aspect of health.

Let’s hit the problem first:  “infertility is not an issue but a medical problem of the reproductive system of the couple and it is treatable.”

Infertility may be due to medical conditions like low egg reserve, low quality of eggs, tube defects, sperm issues, and uterine abnormalities that are proven and that can be checked by various means. A recommendatory blood test for the couple along with sperm analysis is the most advised one.

Desperate trying for pregnancy along with failure to get one is a chaotic condition.

Here’s the truth – IVF is the most effective fertility treatment available, accessible & affordable today, meaning it has the highest success rates, but it is not the only one. There are other options available for pregnancy.

  1. Ovulation monitoring:

 Ovulation monitoring is a natural process with vigilant expert guidance. Having timely intercourse during the fertile window or period is the most effective way to achieve pregnancy. The simplest procedure is careful monitoring of periods to achieve pregnancy and not a treatment as such, but if done in consultation with the doctor, along with a few blood tests, and a vaginal ultrasound to see how close you are to ovulation and can instruct you to have sex with a partner at the optimal time.

This option is helpful for couples, who are not conceiving due to wrong timings of sexual intercourse.

The ‘fertile window’ is different for every lady. The ‘fertile window’ is the day an egg is released from the ovary and the five days before that. During this time, uterus is in receptive form & sperm mating can cause pregnancy.

Ovulation Induction:

The next option is ovulation induction, which is when, you are given oral medication, such as Clomiphene, to help in ovulation. The doctor will do a vaginal ultrasound and predict ovulation, and have timely sex at that time. This medicine-driven ovulation is intended for women having irregular cycles or Polycystic Ovarian Syndrome (PCOS).

This PCOS is a multi-factorial disorder involving many factors & can cause irregular menses.

There are many good medicines available that help women conceive. These fertility medicines are strictly prescription based & should be taken under the guidance of registered medical practitioners.

Intrauterine Insemination (IUI):

One more step in fertility treatment is IUI (Intrauterine Insemination) after ovulation monitoring and ovulation induction. IUI is most helpful for men where sperm motility is a matter of concern. “IUI is the full-proof solution for unexplained infertility, endometriosis, or male factor infertility.”

 In general, IUI does two things:

Medications are given to increase the egg counts, so increasing the chances of her fertility.

Then, via the injection, the sperm is inserted inside the uterine cavity, so it can escape the travelling path.

During the procedure, the male partner is asked for a semen sample about an hour before IUI. Sperms are then cleaned off unwanted matter which, increases fertility chances.

Insemination is usually a few minutes pain-free procedure. The doctor will pick the healthy sperm and place sperm through the catheter into the uterus. This is an almost patient-friendly technique.

In Vitro Fertilization (IVF)

The finest and most widely performed treatment is IVF, which is In vitro fertilization. IVF has the highest pregnancy success rates and is the most advanced and evolved of all treatments.

IVF is specifically for the Women with PCOS, Diminished Ovarian Reserve (DOR), endometriosis, and severe male factor infertility, such as low sperm count and motility.

While IVF is technologically and medically advanced, the treatment itself is not very simple. IVF, in simple terms, is, the union of sperm and egg outside of the body (in the laboratory) which gets developed into an embryo and get transformed into a woman’s uterus as pregnancy.

HOW DOES AGE AFFECT IVF TREATMENT COSTS?

Women’s fertility decreases with age and rapidly so after the age of 35. A woman’s ability to produce good quality eggs reduces as her age approaches 40, which is considered the age around which most women get their menopause. This simply means that the woman has exhausted her complement of eggs, is now unable to produce any more eggs and therefore cannot produce a child. This is accompanied by hormonal changes as periods cease and we say that menopause has started.

However, this doesn’t mean that the woman cannot get pregnant. She can take the services of a donor, generate good quality embryos by artificially fertilizing eggs donated by a healthy woman with her husband’s sperm and achieve pregnancy by getting these embryos implanted into her uterus. This process is called IVF using Donor eggs and is very common. Here the cost of the IVF process increases by about 40k to 50k being the Donor fees.

Age is not a very major issue in men.

 

HOW DO CO-MORBIDITIES AFFECT THE COST OF TEST TUBE BABY TREATMENT

Co-morbidities like Diabetes, Anaemia, Hypertension, Thyroid issues, and Obesity may be present in either/both partners. These ailments may directly or indirectly affect the ability to conceive or bear a child safely. Failure to conceive, premature births, and miscarriages are known complications when such comorbidities are present. Therefore these ailments have to be first cured or brought under control before the IVF cycle can commence. Subsequently greater care, additional medicines, and more frequent monitoring may be required, which may increase the cost of the IVF cycle. WHAT IS THE EFFECT OF THE HEALTH OF THE REPRODUCTIVE TRACT ON IVF COST?

PCOD / PCOS is seen in a vast majority of women facing fertility issues. Depending upon the severity, it may require management ranging from medicines to surgery for facilitating conception and a pregnancy culminating in a healthy birth. In many of these cases, the eggs are numerous but do not mature and cannot generate embryos of sufficient quality. In such cases, pooling cycles may be an answer, but these will increase the cost. A more economical alternative would be IVF with self sperm and Donor eggs.

In men, underlying conditions may necessitate additional procedures to extract the sperm directly from the testes or the reproductive tract. These procedures will incur additional costs varying from 10k to 25k.

Additional treatments, expensive medicines, lab tests, more frequent monitoring, etc in such cases increase the cost of IVF as compared to non-complicated cases.

In women, the ovaries naturally stop producing good quality and number of eggs as they age. In some cases, the ageing is much faster and results in premature ovarian failure. In such cases, depending upon the severity, either expensive medication may be required or Donor eggs may need to be used, thereby increasing the cost of the IVF treatment.

 

From the above, it is important to note that there are several patient-related factors that require additional tests or processes. This increases the treatment cost of the cycle. However, patients must learn to look at IVF costs as an investment in a quality process in order to reduce time and cost in achieving a pregnancy that will culminate in a healthy live birth after a complete gestation.

 

While the above end result cannot be absolutely guaranteed by any clinic, the intent behind the process is more important. IVF TREATMENTS WHO ARE THE EMBRYOLOGISTS?

An embryologist is a reproduction biology specialist. who is in charge of the assessment of eggs, sperm & embryos in order to achieve fertilisation.

Although most of the decisions related to IVF treatments are often taken by the team of doctors, it is up to the embryologists to decide on the specific treatment, as well as coordinating the development of every activity in the laboratory.

 Couples are often advised blood tests to assess their fertility potential. Haemogram, thyroid testing, AMH & other hormone profiles are often done in such cases. Men usually go through semen analysis. Once gynaecologists and andrologists have diagnosed the specific issue, along with senior embryologists, the most appropriate treatment depending on the woman’s age, and sperm quality is planned. Broadly speaking, 3 different approaches can be considered: intrauterine insemination, in vitro fertilization & Intra cytoplasmic sperm injection.

Intrauterine insemination (IUI) is probably the least invasive procedure. It is indicated in tube obstruction, cervical factor infertility or erectile dysfunction, & even some other unclear fertility issues.

IUI requires Embryologists to perform different procedures based on their evaluation of motile sperm that allows for the selection of the best gametes.

Couples suffering from low sperm count or low sperm motility on the male side, damaged Fallopian tubes or endometriosis on the female side are usually recommended to resort to either in vitro fertilization (IVF) or intra cytoplasmic sperm injection. In both cases, ovarian stimulation is required for women. Hormonal stimulation of the ovary increase increases the oocytes produced during the cycle; these oocytes must reach a minimum size in order to mature and be retrieved for the following procedures (either IVF or ICSI. The collection of oocytes is done through laparoscopic transvaginal surgery.

In cases of severe male factors, infertility may require the use of ICSI . This procedure consists of the injection of an individual sperm into the mature oocyte, thus being actually fertilised by the embryologist, one by one.

Sperm selection is based on morphology & motility. 24 hours after the procedure is done, embryologists verify whether fertilisation has occurred by looking under the microscope. The presence of 2 pronuclei is the first sign of fertilisation (one from each gamete), & they are to be fused into a single one in a process called syngamy.

It is of utmost importance to carry out such a procedure with accuracy and precision ,as it’s the transfer of genetic material that develops the baby. Strict hygienic & sterile conditions under specific media and reagents.

Every Inch of the embryology lab needs to be maintained as per the guidelines laid down by authorities. The use of any perfume, or deodorant & alike is completely forbidden just to prevent potential volatile organic compounds (VOS) from affecting developing embryos.

 In the laboratory, the use of caps, masks &disinfectants are mandatory for embryologists. Embryos are quickly placed in incubators that act as the natural environment of the uterus.

The embryology lab is where the time is set according to the procedure. Every second counts as procedures must be done at an exact time point in order to guarantee their success until transfer day.

Routine maintenance of the laboratory & equipment, sperm handling & processing for IUI, IVF & ICSI, cryopreservation of biological material, tissue sampling for biopsies for genetic analyses & PGD, or even embryo culture is of great importance.
